Home Care Packages (HCP) are government-funded programs that support residents over 65 years of age to stay in their own home. These home care packages come in four levels, each designed to cater to different care needs of the elderly. During an aged care assessment, the Aged Care Assessment Team (ACAT) will evaluate your needs or the needs of the elderly in your family and determine the appropriate level of care. This assessment will guide the assignment of your Home Care Package level. How to apply
Step1: Apply for Assessment
Applications are submitted to My Aged Care either online or via calling 1800 200 422 (have your Medicare card handy). Alternatively your GP may refer you for an assessment. Call the Home Care Assistance Sunshine Coast team if you need any help with your application.
Step 2: Attend Face-to-Face Meeting
Once you have applied, a My Aged Care representative will contact you within three weeks to organise an assessment. The assessment will take place in your home to understand your needs, you can access information to prepare here.
Step 3: Await confirmation
Information about current wait times can be found here. When you have received a letter from My Aged Care advising to find a provider, this is a great time to contact Home Care Assistance.
