Contact My Aged Care
Call My Aged Care on 1800 200 422 and let them know you are switching providers. They will reactivate your Support At Home Package referral code, which is essential for your new provider to access your package.
Reach Out to Home Care Assistance
Once you have your referral code, contact our team to arrange your transition.
Our Community Engagement Manager will:
- Confirm the end date of services with your previous provider.
- Set up a home visit to create your personalised care plan.
- Ensure a smooth and uninterrupted start to your new services.
