Annual Health Assessments for Home Care Package Clients
Annual Nursing Health Assessments are recommended for all Level 3 and Level 4 Home Care Package clients. These clinical assessments are conducted annually to ensure clients receive the appropriate level of nursing care, incorporating a comprehensive nursing review of any changes in conditions or situations prior to the annual care plan review.
Post-Hospital Assessments
Our Registered Nurses (RNs) ensure smooth transitions from hospital to home. This includes review of discharge instructions and recommendations for additional care needs.

Comprehensive Health Assessments
Our Comprehensive Nursing Health Assessments cover a range of areas to ensure client well-being and changing needs are always considered and accounted for.

| Medication Administration | Administration of controlled substances, e.g., opioid patches. |
| Injection(s) | Administration of prescribed subcutaneous or intramuscular injections, monitoring for adverse reactions. |
