  1. Complete and print the Queensland Police Fraud Report Form on the Queensland Police website.
  2. Bring the completed form, along with any requested documentation, to your local police station. (A legal professional may assist with preparing your information if needed.)
  3. Once assessed, if the report qualifies for investigation, it will be logged in the QPS case system, and you’ll receive a report number starting with “QP” for future reference.
